Have you experienced something at OAKBERRY that isn't right? Using the button below, employees, customers, partners and others can submit a report or complaint. All reports submitted through the form are anonymous.

The form does not ask for your name, email or any other information about you. Your IP address and other technical information are not sent with the report. If you want to stay anonymous, do not write your name or anything else that could reveal who you are.

Submit a report

Answer as many questions as possible and give detailed information. If you have nothing to write in a field, leave it empty. The report is anonymous.

Who is the complaint about?

Choose a category

Describe it as precisely as possible.

State the nearest location, e.g. the store name, room or department.

State the day and time as precisely as possible.

E.g. talked to someone about it or reported it elsewhere? If so, describe it here.

This does not reveal who you are, but we need it to handle the matter under the laws of your country.

Be precise, and make sure it is the store or department you want to report about.

Attach files (optional)

Attach documentation, e.g. photos, audio files, emails, screenshots or other documents. Note: Make sure the files do not contain information that could reveal who you are. The files are renamed before they are sent.

No acknowledgement or feedback on anonymous reports

We cannot acknowledge receipt of anonymous reports and cannot reply to you. If you would like an acknowledgement or a reply to your report, you can send an email instead, but the report will then no longer be anonymous.
